Summary:
|
In telemedicine and telehealth environment, there are cases where medical or healthcare professionals (e.g., nurses) visit a patient at home and a physician stays remotely and instructs the nurses. This is the so-called “Doctor-to-Patient with Nurses” (D2PwN) model. D2PwN is a hybrid telehealth model where a nurse provides hands-on care at the patient's location while a physician participates remotely via video and/or audio connection. This model combines the benefits of telemedicine with necessary physical examination and procedures. However, in this D2PwN model, patients receiving home healthcare services face significant security risks if the participating individuals are new and unknown to them, and still present themselves as medical staff. Current identification methods are inadequate because of the following:
Medical uniforms are commercially available and don't require verification to purchase
Basic ID badges can be replicated with consumer-grade printing equipment
Agency-issued identification often lacks sophisticated security features
Name tags and credentials can be stolen or borrowed
There may also be issues on the part of patients, such as:
Patients, especially elderly or vulnerable populations, may not know what legitimate credentials should look like
Visual impairment or cognitive issues can make verification difficult
Patients often feel pressured to admit anyone claiming to be medical staff
This creates vulnerability for patients, particularly those receiving regular home care services. One of the sources of the problem is the lack of standardized authentication mechanism for such cases that is used across different healthcare or medical organizations.
This draft Recommendation describes an authentication system that addresses this vulnerability issue telehealth that is combined with visit-home-care. The system provides verification of medical staff identity while maintaining ease of use for patients and healthcare providers.
