11.	Broadcasting frequencies	Permission from Broadcasting Council to broadcast in Uganda (set up radio and/or television station). Engineering brief of the project whose format shall include: Equipment Technical specifications (on Radio or TV transmitters including Studio to Transmitter Link (STL). Studio and Transmitter site linkage arrangements. Broadcast coverage areas. Completed UCCFM-6 and/or UCCTV-7 forms (to be obtained from the Commission offices) . 13.	Customer Services and Internal Block Wiring	Company Profile (to include certified photocopies of the Certificate of Registration or Incorporation of the company). 14.	Two Way Private Radio communication	Completed UCCFM-5 form (to be obtained from the UCC) These forms request for general equipment information. 16.	Aircraft Radio Station	Photocopies of the Certificate of Registration/Incorporation of the company of the company). Completed UCCFM-5 form (to be obtained from the Commission). Photocopy of the Certificate of airworthiness from Civil Aviation Authority. 17.	Use of VSAT (Single user)	Company Profile (to include certified photocopies of the Certificate of Registration or Incorporation ).
